This presentation will help you understand:-Questions to ask Providers, yourself, and your loved ones about your health care-The ways Home Health, Palliative Care, and Hospice can support someone with a serious illness-When it might be the right time to transition to one of these types of care. Co-presenter Bios: Jennine Greenwell is Jackson County’s Older Adult Behavioral Health Specialist. She holds a master’s degree in Human Services. Summer Irwin is the Providence home health and hospice community liaison. She engages with local healthcare providers, community organizations and families to promote awareness of Providence’s comprehensive home health and hospice services.
